REGISTRATION NOTES

Registration fee includes entrance to all unrestricted meetings, morning and afternoon coffee breaks, lunch on September 15 and 16, meeting program, one annual banquet ticket and a certificate of attendance.

In order to qualify for the "Active Society Member" registration fees, your Histiocyte Society membership dues must be paid in full through 2009.

Only active Society members are eligible to attend and participate in Study Group Meetings.

Those registering under the category of "Nurse/Fellow/Student" may be asked to provide documentation verifying this status.

International Family Organizations are invited to register a maximum number of three persons per organization.

Visa, MasterCard, American Express, Discover, and PayPal are accepted forms of payment for online registration.

CANCELLATION POLICY

A refund of 50% will be granted to those who cancel by July 31, 2009. We cannot refund any registrations that are canceled after July 31, 2009.
